Fellowship Request Form
All candidates and future advisors should turn in fellowship request forms.
Candidates for Master’s fellowships and Direct Entry Doctorate fellowships should first take the Fellowship Exam. The dates for the Fellowship Exam will be announced together with the dates for the Entrance Exams.
The Master’s fellowship and Direct Entry Doctorate fellowship candidates who obtain grades greater than or equal to 5.0 (five) on the Fellowship Exam will classify. Fellowship Exam grades are valid for one year and will be substituted for new results if the candidate takes another Fellowship Exam.
Doctoral fellowships are determined through an examination of the necessary documentation provided.
The classification lists for the Master’s/Direct Entry Doctorate and the Doctorate, will be updated monthly.
The fellowships available will be awarded according to the classification list.

If data presented on the forms is not correct or not proven, it is possible that the fellowship request will not be considered.
Since the classification lists are updated monthly, any updates to the forms should be made by the 25th day of any given month for the updates to be considered the following month.
